Information and Waiver Forms

The following are the names of individuals that have your permission to pick up your dog from Paradise Pet Salon Grooming, Daycare & Boarding. You understand that once dogs are released into the care of authorized persons or individuals that Paradise Pet Salon Grooming, Daycare & Boarding is no longer responsible for the well-being of your pet.

Pet Behaviour Profile

Rules and Regulations:

• We accept dogs of all ages. Puppies must be over 12 weeks and on a vaccination program. Puppies or shelter dogs must have been in the home for at least two weeks prior to attending daycare.
• All dogs require up to date vaccinations for rabies, DHLPP, and Bordetella. Proof of vaccination is required.
• Dogs that are currently ill or injured are not permitted at daycare. Dogs that have recovered from a communicable illness within the past 30 days must provide veterinary clearance to return to daycare. Owners certify by signing this waiver that their dog is in good health.
• If your dog incurs veterinary bills during their stay at daycare you, the legal owner of the animal, must pay those bills at time of pickup. Paradise Pet Salon Grooming, Daycare & Boarding reserves the right to seek veterinary care for your dog at its discretion.
• All dogs must be non-aggressive and not food or toy possessive. Owners certify by signing this waiver that their dog has no history of threatening or harming any person or animal.
• We reserve the right to refuse any dog for any reason at any time.
• Pick-ups must be completed by 6:30 PM. Late fees for late pick-ups do apply at a rate of $15.00 for every 15 minutes or part thereof past 6:30 PM. No exceptions.
• In the event that a pet is found to be carrying fleas, it will be given a flea bath at the owner’s expense. ($25-$55)
• When boarding overnight, Owner agrees to supply adequate food and medications for the stay. Should additional supplies be necessary, Owner authorizes Paradise Pet Salon Grooming, Daycare & Boarding to purchase said supplies and agrees to reimburse Paradise Pet Salon Daycare & Boarding for the actual costs plus a trip fee of $25.00/occurrence.
• Paradise Pet Salon Daycare & Boarding will not be held responsible for any items brought with the pet that may be lost, damaged or eaten.

Waiver and Agreement

1. I hereby represent that I am the legal owner of the dog(s) described above and am hereby authorized to use the services provided by Paradise Pet Salon Grooming, Daycare & Boarding.
2. I hereby represent that my dog(s) is of good health and has not been ill with any known contagious diseases within the past 30 days.
3. I recognize that the health of the dog(s) is the owner’s responsibility. I hereby represent that all required vaccinations (rabies, bordetella, distemper and parvovirus) are up to date. I will also continue to ensure that the required vaccinations will be kept up to date for as long as my dog(s) attends Paradise Pet Salon Grooming, Daycare & Boarding. In addition, I hereby represent that my dogs have flea/tick preventative treatments applied regularly.
4. I further understand and agree that in admitting my dog(s), Paradise Pet Salon Grooming, Daycare & Boarding has relied on my representation that my dog(s) is in good health and has not harmed, shown aggression or threatening behavior towards any other person or any other dog.
5. I further understand and agree that Paradise Pet Salon Grooming, Daycare & Boarding and their caregivers will not be held liable for any problems that might develop with my dog(s) including, but not limited to sickness, disease, injury, running away and death, provided that reasonable care and precautions are followed.
6. I understand and agree that any problem that develops with my dog(s) will be treated as deemed best by the staff of Paradise Pet Salon Grooming, Daycare & Boarding at their sole discretion and that I assume full financial responsibility for any and all expenses incurred.
7. I understand and agree that during normal dog play and despite constant, careful supervision, my dog may sustain injuries such as scratches, punctures, torn ligaments and other injuries. I hereby release all staff and owners of Paradise Pet Salon Grooming, Daycare & Boarding from any liability of any kind whatsoever arising from or as a result of my pet’s attendance at Paradise Pet Salon Grooming, Daycare & Boarding.
8. I agree that my dog(s) may be videotaped, photographed and or recorded. Paradise Pet Salon Grooming, Daycare & Boarding shall be the exclusive owner to the results and all proceeds of such media.
9. I understand that Paradise Pet Salon Grooming, Daycare & Boarding reserves the right to permanently remove a dog from care at any time and for any reason to ensure the safety of other dogs as well as staff.
10. I understand that the rules above apply to any dog(s) of mine attending Paradise Pet Salon Grooming, Daycare & Boarding.
